Case Manager

The case manager is the liaison between the patient, family, and the treatment team. The case manager initially evaluates the patient’s and family’s discharge plans and relays this information to the team.

The case manager will provide periodic updates of the patient’s progress and will assist with discharge planning and equipment needs. The case manager is responsible for scheduling follow up doctor appointments and continued therapy services whether it is Home Health or Outpatient Therapy. A successful return home is the primary focus of the case manager and he or she will assist the patient and family in identifying resources in the community.
