Age Range
Cape Fear Valley Rehabilitation Centers admits patients from age 0-100+. Patients under the age of 5 are evaluated on a case by case basis.
Activity Limitation
Patients must be able to tolerate at least 3 hours of intensive therapy services at least 5 out of 7 days.
Behavioral status
Patients must be able to follow the defined patient responsibilities on a consistent basis to qualify for inpatient rehabilitation services.
Cultural Needs
Cape Fear Valley Rehabilitation is attentive to the specific cultural needs of each patient. The rehabilitation team can assist with requests on a case by case basis.
Impairments
Patients to be admitted to rehab should meet one of the CMS diagnostic or disability categories or have an impairment which precludes functional independence with self care, mobility, or communication.
Medical Acuity/ Stability
The patient must be medically and neurologically stable and have the physical and mental capacity and motivation to participate in an intensive rehabilitation program.
Participation restrictions
Patients on mechanical ventilation, receiving intravenous anticoagulant therapy, and/or with active psychiatric disease do not meet the criteria for inpatient rehabilitation at Cape Fear Valley Rehabilitation Center.
Psychological Status
Patients should not require medication, which impairs his/her mental and/or physical ability to participate in rehabilitation. Patients with a current history of substance abuse will not be accepted into the program until substance use has been discontinued and withdrawal symptoms have passed.
Days/Hours of Service
The rehabilitation center inpatient unit operates 7 days a week 365 days a year 24 hours a day.
Frequency of Service
Physical/Occupational and Speech Therapy Services are provided 7 days a week. Recreational Therapy and Psychology Services are provided 5 days a week.
Payer sources
Most major insurance carriers cover inpatient rehabilitation services. If a patient’s insurance does not cover inpatient rehabilitation services, the patient may be admitted on a self pay status with the consent of the Service Line Director or Vice President of Post Acute Care Services.
Fees
There must be a predetermined payment status and an established post rehabilitation disposition planning option.
Referral Sources
Patients may be referred to the rehabilitation center by a licensed physician in the State of North Carolina.
Intended Discharge/Transition Environments
Patients are admitted to inpatient rehabilitation with the goal of being discharged back into the home environment. In some cases, alternative discharge locations may be necessary.
